The Paiute Willow Basket Community Needs Survey window has been extended through next week! Your responses will guide Tribal leaders in improving communications and prioritizing future projects.
Two of the $500 winners have already been announced, with more to come!
What can I still win?
• All participants will receive a $25 ham, turkey, or veggie voucher.
• On March 14, the final winner will be drawn for a prize up to $100 in value.
• Next Friday, March 14, we will draw the remaining four $500 winners.
PLEASE NOTE: Survey takes approximately 30 minutes. Because responses will not be saved in order to protect confidentiality, you must complete it in one sitting.
You must be 18 years or older. To take the survey, have your CIB number ready and scan the QR code or click here.
You can also get a paper survey at the Tribal Admin Building, 440 N. Paiute Drive in Cedar City, or call Anthonia (435-586-1112) to request a personal home visit to assist you.
