The Patient Care Program wants you to know your numbers and take control of your health!

Your A1C and blood pressure are key indicators of your overall health. Keeping them in check can help reduce the risk of diabetes complications, heart disease, and more.

Why It Matters:

A1C (average blood sugar over 3 months) – Aim for 7% or lower (or your provider’s recommendation).
Blood Pressure – A healthy range is less than 120/80 to protect your heart and organs.

Tips to Lower Your Numbers:

  • Follow My Native Plate guidance from Indian Health Services – Fill half your plate with vegetables, one-quarter with a healthy protein, and one-quarter with a whole grain or starchy vegetable.
  • Stay Active – Aim for at least 30 minutes of movement most days of the week.
  • Prioritize Sleep – Poor sleep can raise blood pressure and blood sugar.
  • Hydrate & Reduce Sodium – Drink plenty of water and limit processed foods.
  • Manage Stress – Deep breathing, meditation, or even a short walk can help.

When was the last time you checked your numbers? Talk to your healthcare provider about where you stand and set goals to improve your health.
