Just a friendly reminder that PRC-eligible members need to turn in their 2026 Social Security benefit letter to Member Services if they qualify for payment reimbursement.
Members can upload the letter on our website at https://fourpointshealth.org/documents/, bring them into a FourPoints Health clinic, or provide them to a member of the Patient Outreach team.
