This is the last week to take the Paiute Willow Basket Community Needs Survey! Your responses will guide Tribal leaders in improving communications and prioritizing future projects.
What can I win?
• All participants will receive a $25 ham, turkey, or veggie voucher.
• Each week, a winner will be drawn for prizes up to $100 in value.
• At the end of the survey, eight winners will be selected and win $500 each!
PLEASE NOTE: Survey takes approximately 30 minutes. Because responses will not be saved in order to protect confidentiality, you must complete it in one sitting.
You must be 18 years or older. To take the survey, have your CIB number ready and scan the QR code or click here.
You can also get a paper survey at the Tribal Admin Building, 440 N. Paiute Drive in Cedar City, or call Anthonia (435-586-1112) to request a personal home visit to assist you.
