COMPLETELY ANONYMOUS
PITU is working with the Inter-Tribal Council of Arizona’s TEC to assess the extent of the opioid overdose crisis and identify harm reduction resources among our Tribal members and in or around our Tribal communities.
To help with this, we have designed a community needs assessment survey for adult Tribal members and other PRC eligible adults living within the PITU service area. Responses to the survey are completely anonymous.
As a reward for completing the survey, participants will receive a $30 incentive (via an ACH transfer – instructions will be provided at the end of the survey).
